Botulinum toxin type A is a prescription medicine injected into named facial muscles with a fine 30 to 33 gauge needle. It blocks the signal between nerve and muscle, so the injected muscle contracts less for a few months. The lines it treats are dynamic ones — creases made by repeated movement, frowning, squinting, raising the brows — rather than by lost volume or sun damage, so when the muscle moves less the skin stops folding along the same crease and the line softens. In the United States the licensed cosmetic areas are four: frown lines between the brows, crow's feet, forehead lines, and the vertical bands of the neck. One appointment takes roughly 30 minutes and needs no anaesthetic beyond numbing cream, if you want it.
It suits adults with moderate to severe movement lines in the upper face who want something reversible and non-surgical, and younger patients using smaller doses to slow lines becoming etched in. It will not do the other jobs. It does not treat lines already set into the skin by sun damage or laxity, it does not replace lost volume, and it does not lift sagging tissue — those need resurfacing, filler or surgery, and we will tell you that rather than inject you anyway. We do not treat during pregnancy or breastfeeding, in neuromuscular disorders such as myasthenia gravis or ALS, where there is existing eyelid ptosis or marked facial muscle weakness, where there is infection at the injection site, or where you have reacted to a botulinum toxin product before.

For frown lines, we use 5 small injection points between the brows: 2 on each side and 1 in the centre. The usual total is 20 units. This dose has been shown to soften moderate to severe frown lines.

For the fine lines at the outer corners of the eyes, we use 3 small injection points on each side, usually 24 units in total. The injections stay well clear of the eye muscles. They soften the lines that appear when you smile, rather than removing lines visible when your face is at rest. Temporary eyelid swelling is reported in about 1% of patients.

We treat forehead lines together with frown lines, not on their own. The usual plan is 20 units across 5 points on the forehead plus 20 units between the brows, for 40 units in total. Treating both areas together helps prevent the brows from sitting lower. Headache, reported in about 9% of patients, and lowered brows, reported in about 2%, are the most common reactions.

This treatment softens the vertical bands that can show in the neck and may sharpen the line of the jaw. The dose depends on how visible the bands are, usually 26, 31 or 36 units, given in small amounts along the upper neck and the bands themselves. It can improve neck banding and jaw definition, but it is not an alternative to a neck lift.

For primary underarm or hand hyperhidrosis — excessive sweating not caused by another condition — we inject 50 units into each area across multiple small points. It temporarily blocks the nerve signal to the sweat glands; reduced sweating usually begins within 1 week and commonly lasts several months. This is not a treatment for unexplained new sweating, which needs medical assessment first.

Botox is used preventively for chronic migraine — 15 or more headache days a month, with migraine features on at least 8 days.It can reduce migraine-day frequency over repeated treatment cycles, but it does not treat an acute migraine attack.
The appointment, step by step
Assessment with your face moving
The prescribing physician watches you frown, raise your brows and squint, because the dose follows the muscle that is actually pulling rather than a standard diagram. Your medication list, any neuromuscular condition, and the brand, date and unit count of your last treatment are taken here — each of them changes the dose, and occasionally the answer.
The plan written as units per site
Before anything is drawn up, the plan is recorded as a named product and a number of units at each injection point. Units are not interchangeable between botulinum toxin products, so this is also the only form of the plan that can be compared with another clinic's quote.
Marking and skin preparation
Points are marked against your own anatomy and the skin is cleaned; numbing cream is optional and costs about 20 minutes if you want it. The marking matters most around the eye, where staying lateral to the orbital rim is what keeps toxin away from the muscles that move the eyeball and the upper lid.
The injections
Measured volumes go in through a 30 to 33 gauge needle over roughly 10 minutes; most people describe brief stinging rather than pain. Small raised blebs at each point are expected and settle within the hour.
The first 24 hours
Stay upright for 3 to 4 hours, leave the treated area unrubbed and unpressed for about 12 hours, and skip strenuous exercise for a day — every one of those instructions exists to keep the toxin in the muscle it was placed in. Makeup, screens and normal work are fine immediately.
The 2-week review
Onset runs from day 3 to day 7 and peak effect lands at day 10 to 14, so the result cannot honestly be judged before then. Asymmetry or under-dosing is corrected at that review, which is why who performs and pays for a top-up should be agreed in writing before you are injected.

- First 24 hours
The injection blebs flatten within the hour and makeup can go straight on. Stay upright for 3 to 4 hours, leave the area alone for about 12 hours and keep off strenuous exercise for a full day, all to limit spread beyond the treated muscle. A headache in the first day or two is the reaction most often reported after forehead treatment, at about 9%.
- Days 3–7
The first softening appears — movement in the treated muscle starts to drop, and the crease it makes goes shallower. Any bruise from the needle is at its most visible in this window and fades over about a week; concealer covers it and nothing needs to be done about it.
- Days 10–14
Full effect. This is the point at which the result is assessed and the only fair moment to compare it with your before photographs, which is why the review sits here and not earlier. If a side is under-dosed or uneven, a small correction dose is placed now rather than at the next full session.
- Weeks 3–6
The settled result, with no restrictions of any kind — full exercise, saunas, flights and facials are all back to normal. If a brow or an eyelid is sitting lower than it should, it is most noticeable early and improves as the effect wears off; there is no agent that reverses botulinum toxin on demand, so the treatment is time.
- Months 3–4
Muscle activity returns gradually and the lines come back to how they were before — the labelled duration for frown lines is 3 to 4 months, extending to a median of roughly 6 months when the full 64-unit upper-face pattern is used. Re-treatment is not given more often than every 3 months, because dosing at shorter intervals has not been clinically evaluated.

- Tell us every medicine and supplement you take — aspirin, NSAIDs, anticoagulants, and all herbs, vitamins and minerals. Several of them make injection-site bruising more likely.
- Leave alcohol alone for 24 hours before the appointment; it raises the chance of redness and bruising.
- Tell us if you are pregnant or breastfeeding, if you have any neuromuscular disorder, when you last had botulinum toxin and at what dose, and about any previous facial surgery or cosmetic treatment. All of it changes the dose and the risk.
- Have the consultation with the prescribing physician yourself. Botulinum toxin is a prescription-only medicine and has to be prescribed after direct assessment, not sold from a price list.
- Book at least 2 weeks before anything you care about. Peak effect lands at day 10 to 14, which leaves room for a bruise to fade and for any asymmetry to be reviewed and corrected.

- Eyelid ptosis — a drooping upper lid — when toxin diffuses into the levator palpebrae superioris. Reported in about 3% of glabellar line treatments. It resolves on its own over weeks.
- Brow ptosis and headache after forehead treatment: headache reported in about 9% of patients and brow ptosis in about 2%.
- Bruising, swelling, redness and tenderness at the injection points, with higher bruising risk if you have taken anticoagulants, NSAIDs or alcohol beforehand.
- Eyelid oedema, dry eye or excessive tearing after injection around the eye.
- Unintended weakness in a neighbouring muscle, giving facial asymmetry or an expression that does not read as yours — including difficulty swallowing or neck weakness after platysma injection.
- Distant spread of toxin effect. Every botulinum toxin product carries a boxed warning for swallowing and breathing difficulty that can be life-threatening, reported hours to weeks after injection. It has been seen overwhelmingly at therapeutic rather than cosmetic doses, but the warning applies to the product we use.

This is an appointment, not an operation — no admission, no anaesthetic, nothing to sit out. What makes it different when you travel for it is timing: you will fly home before the result exists, so most of the value sits in what is written down before the needle.
Before you fly
Insist the quote names the product and the number of units, not just 'Botox'. Units of one botulinum toxin product cannot be converted into units of another, so a price 'per area' with no brand and no unit count is not a comparable price. The labelled doses give you the benchmark: 20 units for glabellar lines, 24 units for lateral canthal lines, 40 units for forehead and glabellar lines together. Since April 2025, facilities and intermediary agencies treating international patients in Turkey must hold an International Health Tourism Authorisation Certificate, be reachable in at least two foreign languages 24/7, provide an itemised invoice of services, unit prices and medications free of charge, and carry liability for complications arising from the treatment. Ask to see the certificate and the itemised quote before you pay a deposit.
At the clinic
The assessment happens with your face moving — frown, raise, squint — because the dose follows the muscle rather than a diagram, and the plan is written in units per site before anything is drawn up. The injection itself takes about 10 minutes with a fine needle; most people describe it as brief stinging. You leave with small raised bumps at the injection points that settle within the hour, and makeup can go on before you reach the car.
Recovery in Istanbul
Allow 2 days in Istanbul and 1 hotel night. There is nothing to recover from — no dressing, no stitches, no restriction beyond staying upright for 3 to 4 hours, leaving the treated area alone for about 12 hours and skipping the gym for 24. The extra night is not clinical caution. It is a window: if a bruise or an unexpected reaction appears, we would rather look at it in person than at a photograph of it.
Heading home
There is no flight restriction after botulinum toxin, because there is no operation behind it — no incision, no general anaesthetic, no immobility, no drain — so none of the clotting reasoning that governs flying after surgery applies. A late-afternoon flight after a morning appointment already meets every aftercare instruction; we prefer you fly the next day for the same reason as the extra night. The result will not be visible when you go. Onset is day 3 to 7 and full effect is at day 10 to 14, so agree in writing before you are injected how the 2-week review will be done remotely and, more importantly, who pays for and performs a top-up if the result is under-dosed or uneven. A return flight for a 5-unit correction is the largest hidden cost of treating abroad.

How long does Botox last?
About 3 to 4 months for frown lines, which is the duration stated on the FDA label. A 2023 meta-analysis of randomised trials found that treating glabella, crow's feet and forehead together at the full approved 64-unit cumulative dose extended median duration to about 182 days. Your own duration varies with dose, muscle bulk and metabolism.
How long does Botox take to work?
Most people see the first softening between day 3 and day 7. Full effect is reached at about 10 to 14 days, which is why review and any top-up dosing are scheduled at 2 weeks rather than straight away.
Is Botox safe?
Cosmetic doses are small next to the therapeutic doses used for conditions such as spasticity, and a meta-analysis of 7 placebo-controlled trials found no significant difference in adverse event frequency between botulinum toxin and placebo. All products carry a boxed warning about distant spread of toxin effect; the serious events behind it have been associated with much higher doses and with underlying medical conditions. The American Academy of Dermatology notes that no serious side effects such as breathing or swallowing problems have been reported when dermatologists use it for signs of ageing.
Will my face look frozen?
Not if the dose is matched to your muscle. The aim is less movement in the muscle that makes the crease, not a face that cannot move — you should still be able to look surprised, and annoyed. If you have had a result you disliked before, bring the brand and the unit count from that treatment; it is the single most useful thing you can tell us.
Is there any downtime after Botox?
Effectively none. You can apply makeup as you leave the treatment room and go back to work the same day. The standing instructions are to stay upright for 3 to 4 hours, leave the treated area unrubbed and unpressed for about 12 hours, and skip strenuous exercise for 24 hours.
What happens if I stop having Botox?
Muscle activity returns gradually and the treated lines come back to how they were before. Stopping does not leave the skin worse than baseline. Long-term repeated cosmetic use has not identified significant safety concerns.
What is the difference between Botox, Dysport, Xeomin and Jeuveau?
All four are FDA-approved botulinum toxin type A products working by the same mechanism, differing in formulation, accessory protein content and unit potency. Units are not interchangeable between them, so a quote saying '30 units' without naming the product is not a price you can compare with anything. Which one is used is usually a matter of injector preference and how you have responded previously.
Can Botox fix all wrinkles?
No. It treats dynamic lines produced by muscle contraction — frown lines, crow's feet, forehead lines and platysma bands are the four cosmetic indications on the US label. Static lines from sun damage, skin laxity or lost volume do not respond and are addressed with resurfacing, fillers or surgery instead.
Who is allowed to inject botulinum toxin in Turkey?
Only a physician, in a Ministry-licensed health facility. A 2015 Ministry of Health circular sent to all 81 provinces stated explicitly that people outside the medical profession cannot perform these procedures on the basis of course certificates. A January 2025 amendment widened the eligible physicians beyond dermatologists, plastic surgeons and medical-aesthetics certificate holders to doctors working within the competence their training gives them — so asking specifically for a dermatologist or plastic surgeon is a question about training, not about legality. Beauty salons and non-physician injectors remain outside the law.
Does facility accreditation mean the injection will be good?
It tells you about the building, not the hand. For this procedure the variable that decides your result is the person injecting: reported eyelid ptosis after glabellar injection falls from a range of about 1 to 14% to under 1% in series from experienced injectors, and diplopia sits below 1% in experienced hands.
Who should not have Botox?
We do not inject during pregnancy or breastfeeding, in neuromuscular disorders such as myasthenia gravis, ALS or Lambert-Eaton syndrome, where there is infection at a planned injection site, or if you have reacted to any botulinum toxin product before. Medicines matter too: aminoglycoside antibiotics, muscle relaxants and anticholinergic drugs can potentiate the toxin, and a recent dose of any other botulinum product adds to your cumulative total — which is why the consultation starts with your full medication list and the date, brand and unit count of your last treatment. An eyelid that already droops or a brow that already sits low is not an automatic no, but it changes where the dose goes, and sometimes whether it goes in at all.
Which symptoms after the injection need urgent attention?
Trouble swallowing, speaking or breathing, new hoarseness, muscle weakness or drooping away from the treated area, blurred or double vision, or loss of bladder control. These are the signs of distant toxin spread named in the boxed warning, they can appear hours to weeks after injection, and they call for emergency care where you are — not a message to us first. They are very rare at cosmetic doses. Everything else — a bruise, a heavy brow, a lid sitting lower, asymmetry you notice in the mirror — is not an emergency: photograph it in good light and send it to us, because it decides what happens at the 2-week review and whether a correction dose is placed.
Will Botox stop working if I keep having it?
For almost everyone, no — repeated cosmetic treatment does not wear out the response. A small minority develop neutralising antibodies to the toxin and stop responding, and the risk of that rises with higher doses and shorter intervals, which is one reason the label sets a floor of 3 months between treatments and why chasing a fading result with an early top-up is a bad trade. What is far more common than true resistance is a session that used fewer units, a different product whose units are not equivalent, or injection points placed differently — which is why the brand and unit count from every treatment is worth keeping.
